Nursing home or home care, indication
What is it?
In most cases, your family or a home care organization can assist you with your household and personal care. If that is not possible, you may be able to obtain a place in a nursing home. To do so, you will need an indication. An indication is a decision that states what care you need and whether you will receive it. You can obtain this indication from the Care Assessment Center (CIZ).
What should I do?
At the CIZ, you can check whether you need an indication for the care you require. You can apply for the indication yourself. However, you can also have the application submitted by a professional healthcare provider, such as a home care agency or your family doctor. In urgent cases, a doctor will always submit the application.
What do I need?
- identity documents for you and your partner, if applicable
- your social security numbers and those of your partner, if applicable
- proof of your income and that of your partner
How long will it take?
You will be notified within 6 weeks whether you will receive the indication.
