Nursing home or home care, eligibility
What is it?
In most cases, your family or a home care agency can help you with household chores and personal care. If that is not possible, you may be able to get a spot in a nursing home. To do so, you need a care assessment. A care assessment is a decision that specifies what care you need and whether you will receive it. You will receive this care assessment from the Care Assessment Center (CIZ).
What to do?
You can check with the CIZ ( ) to see if you need a care assessment for the care you require. You can apply for the assessment yourself. However, you can also have a professional care provider submit the application on your behalf. For example, a home care agency or your family doctor. In urgent cases, a doctor will always submit the application.
What to bring?
- proof of identity for you and your partner, if applicable
- your social security numbers and those of your partner, if applicable
- proof of your income and that of your partner
How long does it take?
You will be notified within 6 weeks whether you have been approved.
