Affidavit or sworn statement
What is it?
The municipality maintains records of its residents in the Personal Records Database (BRP). This information must be accurate. For this reason, the municipality obtains this information from official documents, such as birth certificates, marriage certificates, or divorce decrees.
You can make a sworn statement or give a pledge to the municipality if:
- your information in the BRP or missing, and
- You do not have any other document to verify your identity
Difference between an affidavit and a sworn statement
If you are a person of faith, you should make a sworn statement. You should conclude with the words: “So help me God.” In all other cases, you should make a promise. You should conclude with the words: “I promise.”
What should I do?
- Please contact the municipality to prepare the sworn statement or affidavit.
- The municipality will first check to see if there is another document that contains your information.
- If no other document is available, choose whether you want to make a sworn statement or a solemn affirmation.
- You must make a sworn statement or take an oath at the municipal office.

- documents that specify what information is involved (if you have them)
