Change of last name
What is it?
- If you want to change your last name or your child’s last name, first check the Justis brochure “Name Change” ( ) to see if this is possible. Justis is the screening authority of the Ministry of Justice and Security.
- If your situation is described in this brochure, you can submit a request for a name change to Justis.
What should I do?
- Download the application form at and fill it out.
- Check the " Name Change" brochure to see which code applies to your situation. Enter this code on the application form. Enter this code on the application form.
- Attach the required documents to the form and send everything to Justis.
You will be notified automatically if your last name is changed. You will then receive an official copy of the Royal Decree.
Editing documents
- If your last name has been officially changed, it must also be updated on your identification documents. Therefore, apply for a new passport, ID card, and/or driver’s license.
- Be sure to notify all relevant authorities of your name change. You do not need to notify the municipality separately; it will automatically receive a copy of the Royal Decree.
How long does it take?
You will be notified via Justis within 20 weeks whether your last name will be changed. This may take longer if your application is incomplete.
File an objection
If you disagree with the decision, you may file an appeal with Justis within 6 weeks.
