Name change surname
What is it?
- If you want to change your surname or that of your child, first check whether this is possible in the Justis brochure on name changes. Justis is the screening authority of the Ministry of Justice and Security.
- If your situation is described in this brochure, you can submit an application for a name change to Justis.
What should I do?
- Download the application form and fill it in.
- Check the Name Change brochure to find the code that applies to your situation. Enter this code on the application form. Enter this code on the application form.
- Attach the requested documents to the form and send everything to Justis.
You will be notified automatically if your surname is changed. You will then receive an official extract from the Royal Decree.
Modifying documents
- If your last name has been officially changed, this must also be changed on your identity documents. You will therefore need to apply for a new passport, identity card, and/or driver's license.
- Also make sure you inform all authorities of your name change. You do not need to inform the municipality separately. They will automatically receive an extract from the Royal Decree.
How long will it take?
You will receive notification via Justis within 20 weeks as to whether your surname will be changed. This may take longer if your application is incomplete.
Lodge an objection
If you disagree with the decision, you can lodge an objection with Justis within six weeks.
