Youth services for young people with disabilities
What is it?
Youth services for young people with disabilities are available to children up to age 18 who are unable to cope with their disability, or who have difficulty doing so. These services, which we refer to as support, are designed to help your child become as independent as possible.
How does it work?
The municipality can provide support on an individual basis or in a group setting. Your child will then receive assistance at home or at a facility. Support can also be provided to people in your child’s social circle. Support is often part of a comprehensive package of services. For example, transportation assistance if your child needs to travel to a day program at a different location.
Support is available for children with:
- a (mild) intellectual disability
- a sensory impairment (such as deafness or blindness)
- a physical disability
- a somatic condition (such as a chronic illness)
- a psychiatric condition (mental disorder, including autism)
Support can be arranged in the following ways:
- by a provider with whom the municipality has a contract. We refer to this as care in kind. We pay the provider directly for the care provided.
- You arrange this yourself by purchasing care from a provider with whom we do not have a contract. You will receive a sum of money for this (known as a personal budget), which is then managed by the SVB. When determining the personal budget, the rate we have agreed upon with a contracted provider is taken into account.

- Please describe your problem.
- A VraagWijzer counselor VraagWijzer an appointment with you for a visit at your home. During this visit, we will discuss your problem in more detail and work with you to find a possible solution.
- You will receive a report (personal support plan) that you should sign, add any comments, and return to VraagWijzer.
- This means you have immediately submitted a request if the report states that the municipality is responsible for providing a solution (or if you have indicated that you disagree with the solution outlined in the report and believe that the municipality should provide a solution to your problem).
You need your BSN for your Wmo application
Would you like to apply for assistance or support under the Wmo? If so, we will need your BSN. Your BSN is your Citizen Service Number. This is a personal number issued by the government.
- We ask for your Social Security number to verify your identity and to process your application properly.
- You must provide your Social Security number when you submit your application. You can do this on the application form or during a conversation with a staff member.
- We handle your information securely and with care. We will use your Social Security number solely for this application.

How long does it take?
Once you have submitted your report to VraagWijzer processed as soon as possible. There are currently delays, so you will have to wait longer than usual. After we receive the report, which includes your request for a municipal solution, we will notify you as soon as possible whether you are eligible.
