Assisted living (Wmo)

What is it?

Supported Living is intended for vulnerable residents over the age of 18 who, due to (highly) complex mental health or psychosocial issues and (severe) difficulties managing their daily lives, are unable to live independently. In Supported Living, you live in a facility that provides supervision and support. The Municipality of Nijmegen is responsible for access to Supported Living. This is regulated by the Social Support Act (Wmo).

"Safe at Home" is designed for vulnerable residents over the age of 18 who, due to serious mental health or psychosocial issues, require in-home support, 24/7 availability and/or accessibility, and the support of the group to enable them to live independently.

How does it work?

Depending on the nature of your situation, you may be eligible for Assisted Living or Supported Living. The municipality of Nijmegen serves as the central municipality for our area. The Gelderland-Zuid Public Health Service (GGD) is responsible for assessing eligibility for Assisted Living or Supported Living. They will determine whether you qualify based on your circumstances.

What should I do?

  • A VraagWijzer counselor VraagWijzer an appointment with you for a visit at your home. During this visit, we will discuss your question in more detail and work with you to find a possible solution.
  • You will receive a report (personal support plan). Please sign the report and add any comments you may have. Then return the report to VraagWijzer.

Does the report state that the municipality is responsible for finding a solution? Or have you indicated that you disagree with the solution outlined in the report and believe that the municipality should provide a solution to your problem?

By returning the report, you have automatically submitted an application.

You need your BSN for your Wmo application

Would you like to apply for assistance or support under the Wmo? If so, we will need your BSN. Your BSN is your Citizen Service Number. This is a personal number issued by the government.

  • We ask for your Social Security number to verify your identity and to process your application properly.
  • You must provide your Social Security number when you submit your application. You can do this on the application form or during a conversation with a staff member.
  • We handle your information securely and with care. We will use your Social Security number solely for this application.

How long does it take?

Once you have submitted your report to VraagWijzer processed as soon as possible. There are currently delays, so you will have to wait longer than usual. After we receive the report, which includes your request for a municipal solution, we will notify you as soon as possible whether you are eligible.
