Your child wants to apply for their own identity card or passport. When do you need to give your consent?

  • Identity card if your child is younger than 12 years old
  • Passport if your child is under 18 years of age

Your request

  • Fill out this form completely and take it with you to the municipality. Make sure that your signature matches your ID.
  • Come along with the child(ren) who wants their own passport or identity card. You will need to make an appointment for this.
  • Bring your original, valid ID. Also bring the other parent's ID (if applicable).
  • Did your child already have their own passport and/or identity card? If so, please bring those documents with you as well.
  • Bring a good-quality passport photo that is no more than 6 months old at the time of application and meets the passport photo requirements.
